  5. College Leaving Certificate (Original & Xerox) - Mandatory
  6. Internship Completion Certificate (Original & Xerox)
  7. Medical Council Registration MCI or Bihar (Original & Xerox)
  8. First Professional to Third Professional (Part-1 & Part-2) MBBS Marksheets (Original & Xerox)
  9. Final MBBS Passing Certificate (Original & Xerox) 
  10. Matriculation or 10th Certificate (Original & Xerox)
  11. Cast Certificate for Reserved Candidate (Original & Xerox)
  12. Relieving letter & Charge Report with countersigned (If in any Service) (Original & Xerox)
  13. Four Passport size Photographs self-signed (as Admit card)
  14. Resignation/Relieving and No Dues Certificate from Previous Course (if any)
  15. Bond for PG student vide Health Department, Govt. of Bihar order no. 450(1)/H, Patna dated 15.04.2017

  9. Character Certificate for institution last attended
  10. Migration Certificate
  11. Residential Certificate.
  12. Caste Certificate
  13. 10 (Ten) Passport size photographs.
  14. Affidavit Regarding gap (Those candidates who have passed 2 years before or earlier and not taken admission in any stream in anywhere).
  15. Hard copy of online filled-in Anti-Ragging Undertaking through www.antiragging.in or www.amanmovement.org.
